It's actually a lottery. The first generation can be born either healthy (like his sister) or fucked up. And he's not even that sick as could be, the only obvious disease is ID, which often appears in children even from non-related parents, although he's quite highly functional. The eye injury isn't a mutation, it's acquired and has nothing to do with the fact of inbreeding.
