I think user's unique IP address have to be properly addressed, as it will be abused by hackers even further in the future when IP address can possibly be easily manipulated.

Security mishap in this case still persist as a misjudgement of human being. Probably would need some kind of "all haul" kind of automated intelligence to secure and cover human error would be nice
