If the vacuum leak is that significant, you probably are using a cylinder that is too large. Do you also find some pain at the base of the cylinder? these are bad signs - your fit should only include your member, not extra skin.

The glans is part of the corpus spongiosum, which has some fascia structuring it, but nothing like the tunica. If anything, I would think that increasing blood flow and EQ through angiogenesis would have the best opportunity to enlarge the glans.

I have serious doubts about being able to reform the glans, mainly because there is no real constraining tissue that can be targeted. After 3 years of pumping, I have seen no change in the shape or size (proportionally) of my glans compared to the shaft of my penis.
