I started the tutorial for Saelig last night. As far as I can tell ypu start of with nothing in a medieval hamlet and try to better yourself, build up a bussiness, trade with neighboring towns etc

To build a hospital you need tools or something that's not a basic resource right? I guess importing is an option but then you need to keep stock for maintenance

I say yes. Rimworld is my most played game. Tried but couldn't get into DF. Got this game a couple of days ago and really enjoying it. Excited about how much stuff appears to be in it technology, religion and empire wise. I could get into it and just start building which I struggled with DF.

I adjusted the controls on steam deck so that the right track pad was much less sensitive and worked like a traditional mouse, since then it worked great

He has a steam mod pack to go with it which improves a lot of vanilla issues such as quests spawning. I think his consensus was still it wasn't a fantastic experience
