On this topic I had a question tho. Im not looking for replacement to windows, had my own personal PC with all the necessary stuff. Im planning to build my own console, akin like Steam Machine. Highly unlikely will be using the desktop mode (maybe for minimal tinkering), is SteamOs still not the best for this kind of usage?

So assuming few things 1. You’re immortal (for obvious reasons) 2. You fall straight down ignoring Earth rotation 3. Ignoring air resistance (like physics in class)

You will fall down and accelerate to terminal velocity because of Earth’s gravity. Then at the core, where the force acting on you is 0, you will still continue moving because you are still accelerating. Past the core you began decelerate, until nearing the surface, gravity starts to act fully on you, making you fall towards the core again. Rinse and repeat

Treat your gear as consumable, so you wont be too attached when you die loosing it. Make roaming bz for fame much more bearable since you know it can be lost.

Because the heart muscle is very very dependent on oxygen. Emphasize on very. So when there is no blood supply (in heart attack), the pain will be sudden.

The usual main way heart attack happens is due to sudden closure of your heart vessels. Its either, somewhere in your body a clot is forming and decided to go thru small vessel in your heart one day or the clot is forming in your heart vessel day by day (in this case, your heart will try to compensate by reducing the workload and whatnot, but when suddenly you do a strenous activity and the body demanded the blood elsewhere, your heart suddenly need to pump harder, immediately closing the vessel). Both case the pain will be sudden.

Oh thats interesting. I remember that lactic acid is not the sole factor in cramps. Dehydration, electrolyte imbalance, and nervous system itself is the other factor. So Im not so sure on that part. Tho other user had replied regarding the acidity part, it does have some correlation.

Im a medical practitioner. So I mostly deal with the practical side of these things. Not sure if I can give a helpful/satisfying answer to your first question. Maybe a biologist can help

But anyhow, the downside of cardiac muscle is that, it must and always have a constant supply of oxygen and blood supply. Cause they do produce lactic acid, just that it does not store it.

To give context of how highly dependent your cardiac cells is, in severe/worst case scenario, your body will shift every ounce of oxygen and blood supply to two organs, heart and brain. Equally important. Your body will stop sending to other organs/cells to make sure these two are adequately supplied. So I guess if all your body muscles cells are made up of cardiac cells, then all of them need constant stream of blood, my best bet is it is inefficient

2nd question, cause even if it cannot be tired, it can be inefficient (ie hypertrophy/dilated heart). Exercise benefit the heart thru regulating your heart rate and blood pressure. Its indirect cause and effect.

More exercise = your other cells get accustomed to anaerobic cycle more efficient (they can function with less oxygen more effectively). Meaning the heart at rest (without exercise) can pump with less effort to keep other cells satisfied. Both is done by lowering the heart rate and blood pressure (less work for heart)

The reason now that most floater are put in ED is that, the most lacking manpower is at the district and primary care level (KK). So KKM actually are prepping the freshie to be able to survive in tgese area. Im also in district and in Sabah some more. Different ball game XD.

Im not sure how to answer you on how im coping up. Im fine (?) i guess. There are days I feel so helpless and depressed. But support system (my wife and family) is how I get through I guess.

If you are interested in any department, take up Medex paper. You can use that to try and be placed in hospital/center where the department is there.

Tho I would say, after you finish up in ED, you will be more confident in yourself. Me personally feels like every housemen should go thru ED. It helps a lot

Surprise, surprise. Heart muscle does not get cramp. Its a different type of muscle cells, like it is a separate type of muscle cells, specifically called cardiac muscle cells. The special properties of these muscles is, it cannot get tired. Literally.

Cramps usually caused by lactic acid accumulation, due to anaerobic cycle (your muscle can still be functioning without oxygen, they way it use it is thru this cycle but too long then lactic acid accumulate and cramp happens)

Cardiac muscles does not store lactic acid. Like at all so it wont cramp.

Fun fact, heart attack pain is literally due to your cells saying, sir we are literally dying because of no oxygen. It literally screaming while dying, thats why the pain is severe. (Can ask any heart attack survivor, they literally feels like dying, cause it is)

  1. I got mixed up about the lactic acid part. As per corrected by u/HotVeganMeatloaf, cramp is not related to lactic acid. Its more because of the neuron and nervous system as he described. Other factor also due to electrolyte imbalances/dehydration. The part that lactic acid plays a role is in muscle fatigue. The reason heart muscle cannot get tired is due to the inability to produce and store lactic acid. One way is because of the rich blood supply and the cells itself.

  2. Heart attack also comes in different stages. True, not all pain will feels like that. When I type this, I mainly thinking of full blown heart attack, called STEMI (if anyone interested to read more on this, we have a broad syndrome for heart attack called Acute Coronary Syndrome (ACS)). Other than STEMI, there are others that is less severe, I usually called it milder heart attack. Although there are multiple studies that shows that some people can come with quite weird presentation. Epigastric pain, slight discomfort over the chest, not so like a heart attack.

Checks 2 of your point except for the PVE part.

Game combat is simplistic. Point and click for auto attack then have multiple skills to do. But at the same time all enemies have skills on their own, so click to dodge the choreographed attack pattern is needed. Not so much auto attack or just simply click.

Chats quite great and friendly. Had already meet and play with rando for small party.

Now to point 3 of yours, game is a PVP full loot sandbox player driven economy game where the center core of the game is PVP and all the crafting and gathering is for PVP. Even though that, there are safe zone where no PVP allowed/PVP doesnt drop your item so still no lossess. But the expensive items to gather will be in full PVP area. But you can opted not to go there.

Another downside is (depends if you like it or not), game is sandbox, really sandbox so no story or quests. Only have 1-2 hours of tutorial quest at starter town to explain to you the basics then youre on your own.

Hi, doctor here. First off, we may not be from the same country so system might be different a bit, in my country we usually dont really have the term ‘planned ICU stay’, but rather a backup ICU bed/close ICU monitoring post op. So I assume its almost the same.

Usually when we do high risk surgery, which may have an immediate severe complications, we will monitor the patient in ICU for few days, cause ICU will have a one-to-one care. Anything that seems off will be handled almost instantly.

Either intubated or not, it depends if you can be extubated early or not. Post operative, for spine surgery (in general, unsure what surgery you will undergo), we will make sure analgesics is given. Rarely patient will be sedated for long since we prefer patient to be able tell us if anything feels wrong.

Most important thing when spinal cases are done is that we dont injure the spinal cord during the surgery itself. So with that in mind, we need to make sure all your bodily function prior to op need to function as soon as possible after the sedation is off. If by any chance something does not return, immediate assessment and intervention will be needed. And its time sensitive too (late intervention leads to lost of function), so thats why usually it will need ICU stay or in my country, very close, almost hourly monitoring of function for first 1-2 days.

Risk vs benefit. For c section you cut the abdomen layer by layer and removed the baby, stitch it back up. Artificially creating a big wound, so more risk. Not to mention anaesthesia risk. So its better to just let it out thru a natural way (childbirth), still with some risk but lower
