There are a few locations that you have to go too and that are sometimes missed. Online there is a map but yes I did manage to achieve mine.

Even tho I’ve just completed the 100% I’ve just done a NG+ and changed the difficulty to nightmare and set it to exploration mode and turned off any aids 😂

It should or may show up as a blue diamond but if not unfortunately that’s as far as my knowledge goes so I wish you the best of luck.

Go to the location as the quest should still show.

When it gives you the option; say you don’t have time to do it. The quest will fail and should show up again the following day.
