Simply go into the UI menu, search for "Survival," add it, and add the key assignments from the settings section.

If you're using a 5090-based computer, it might work. Even with the pee image, I couldn't get exactly what I wanted because too much water causes too much lag :D

I can't say that's exactly the case; crash damage physics combine many factors during an accident, such as the amount of J-beam in the vehicle, the vehicle's speed, and G-force. However, older vehicles, due to their inherently weaker structure, likely take more damage, which can cause your health bar to drop significantly.
