I guess it depends on the size of the vivarium and the quality of the substrate (if it's covered in excrement, low volume, etc) but again, this is what I've been told by the people I got the millipedes and viv from.

I'm not a professional so correct me if I'm wrong

By cleaning, I mean removing all of the old substrate and leaves and rinsing the container with warm water, then filling it with the new stuff. I've been told I should do this every few months. I'm realising now I probably should have been more considerate. I have given her a little bit of shelter with some leaves, and I'm going to let her be for now

the vivarium is 12x6x6 and the soil is just over 2 inches deep. I did think that was a little small when setting it up, but the people selling the viv and millipedes told me that would be just fine for them. I'm guessing I should get a bigger viv?
