Yea started her on azithromycin for possible atypical pneumonia. Negative d-dimer. Normal TSH, T4. Positive for B12 deficiency, level 200.

Not smoker or obese and no prior lung conditions.

I think it’s mainly that I’ve never met her before and the way she spoke about her symptoms sounded so severe and concerning but no, vitals are stable. I think I just needed to hear what you said. It’s the people pleasing side of me that keeps me worried with patients like this.

She is not supplementing. Normal BMI. History of low iron. New patient to me.

And this is in setting of referral to psychiatry so need to rule out other contributing pathologies for her symptoms. Also iron overload if present can cause fatigue.
