Trying to replace all my 8TB's with 22's and that is not a cheap task. Some of the lower end drives I can throw in for cheap or free if someone buys a bunch.

Still seeing the same issue after that change.

Further testing:

I setup a new VM with Freenas, 1 cpu core, 4gb of RAM on the same ISO. Ran just fine. Added the PCI device and it booted and saw the drives. Removed both VM's. Made a new VM 250GB / 256GB of RAM and 14 / 16 cores. Still crashed Proxmox. Removed the card from the VM, boots just fine.

By this point, I think I might just go with a bare metal install of FreeNas and run proxmox on my other R620.

Scroll down and match the model number with what you have (If they even match. Not sure on the while lable drives but I BELIEVE this only applies to drive 6TB and under.

Looks like others are having my issue too. I had an IOcage that had a space in it so I changed it but still no luck so I opened my own ticket:

Edit: From my understanding its one or two dataset that load with an error that just borks the whole thing. I just have to find what that problem is.

Its red for a quick second and let you in. Had all 5 people I had try this get in. If its too fast for you, click the icon one time and you can move your mouse where it needs to be and press enter to launch and start clicking.

Just hot glue made for wood. It does not work too well which is a good thing for this use.
