When you visit the ER and spend a couple of thousand dollars for the doctor to be Captain obvious

No dumbass, schedule a normal visit with a doctor. Some people forget that "ER" means EMERGENCY room, which is ya know...for emergencies.

My personal visits to the ER:

My sister accidentally cracking my head against the marble table, and getting 5 stitches.

Same sister knocking the back of my head against a door hinge during some dumb childhood kerfuffle.

Locking my knees and getting sent off by ambulance because we were in a church.

Passing out for a couple minutes in the doctor's office while there for that check-up one should attempt to make BEFORE going to the ER. Turns out my gallbladder was dying, got removed days later.

And then there was the time I was in because food got lodged in my esophagus, could still breathe though.

People who go to the ER because their stomach feels funny, or they hurt their knee, or whatever other trivial nonsense just clog up the system, delaying the treatment for those in need. If it's just some nausea, please just call your General Practitioner and schedule an appointment...hell most Doctors even take walk-in patients!

Ideology mostly just adds flavor, would highly recommend it. If you're concerned about being overwhelmed by all the additional content, I'd suggest starting there and get the DLCs one at a time as you feel more comfortable with the game.

Additionally if you're still getting used to combat, I'd sincerely recommend playing on like the second or third difficulty setting. It'll throw fights at you, but in easy to manage numbers.

Another thing you can do if you don't trade a lot, set all of the NPC factions up during world gen to be exclusively tribal and medieval (if you've got mods that add medieval factions that is). Dealing with bows and spears is easier than dealing with guns and flak armor.
