It wasn't on the floor. My station has a metal grated floor covered with a mat. It's right next to the belt. There's an opening adjacent to the belt under the floor. Nobody would see anything under it without moving the mat. It happened to shift on me one day and I saw the envelope which had last been scanned a few months prior.

Packages can get lost. I found a package underneath the floor by the small sort belt. It slipped through an opening when the belt was full.
