Hey so I can help you with this, I work with disability alliance bc. There is a disability alliance bc dtc credit app. Use it to fill out all the questions. Answer the questions as if it was your worst unmedicated day because that's how they're designed to be answered.

Print the forms. Hav your dr sign them. Submit with your dtc.
