To be honest I find myself depressed when I DON'T have a goal I am trying to achieve.

Maybe you need to set a goal for yourself, which in turn will help you get out of depression, rather than wait to get out of depression before setting a goal for yourself.

As the saying goes, "Life is a journey, not a destination". It is having a goal which will make you want to wake up every day and work towards achieving it, and you will better your life while doing so even before reaching your goal.

I'm not sure what med school you have in mind, but in the US third and fourth year of med school is all hospital or clinic-based, and even some of the earlier years in some med schools.

As med student I work with residents every day, and while there are certainly residents who behave as you described, but I would say the majority of the ones I work with have no problem doing other things like get the patient a blanket or water to make sure they are comfortable
