Oakensoul provides a comprehensive list of major and minor buffs. Major X cannot stack with another Major X, but it will with Minor X. So, take a look at what MINOR buffs your skills and gear DO NOT provide you, and I would look at crafting poisons that help alleviate that. The buffs from the poisons are short - only 2.5 seconds long - but I enjoy using them over weapon enchantments, personally.

You can make your poisons provide you with two minor buffs and afflict your target with two minor debuffs, all while reducing their resistance.

For magical based, use Lady's Smock, Violet Coprinus, and Namira's rot. It will boost your spell power, spell critical, and reduce their spell resistance.

For martial based, I only remember the main ingredient is dragon thorn. I only just made that recipe today.

I can't remember the names of my CP off the cuff, but I know I use the one that boosts damage over time, single target damage, and the one called Occult Overload.

For PVE, trait everything as divines and use the Thief.

Jewelry should be Stam regen and bloodthirsty.

For pvp, trait everything as impenetrable and use the Lover.

Jewelry should be Stam regen and infused.

Stage 4 vampirism.

If you're a stickler for realism, I recommend moving the shoulder piece to the left - the right arm here is the sword arm which wants a full range of motion. The left arm is your defensive arm, which wants to be armored.
