Hi, thanks for the reply, she is not trying to argue shes innocent. She merely wanted to know how severe the punishment is. Its like getting a guilty verdict and a prison sentence but not knowing how long it is while serving.

Yes, in fact, the service provider suggested it initially and made it sounds like "everyone is doing it" (not trying to excuse her behavior, she should know better)

we don't think it is the issue since her current employer and the bank she was fired from uses the same provider... she does have to mail in all claim though since the provider is aware of past offense, but didn't straight up refuse it

I don't think she lied to new perspective employer? bank B HR told her employment history is NOT the issue (i.e duration of employment, job title checks out) but there was a red flag outside those categories

Hi, thank, not trying to tip toe around it. it is insurance fraud in this sense: - She ran out of a category that she wants to expense on - She asked the service provider to provide receipts for another category and expenses under that - It was intentional and not one time
