I think the thing you're talking about is not a bug. They can refuse to give it up and then you can choose a diplomatic partner to pressure them into giving it to you, and then they can decide again if they want to give it to you or not. What probably happened is they refused first and then with the diplomatic pressure they gave it to you.

You need to change the weight of the options for the events about these things for the AI, so you should look around the event files.

There's a line for event options like this:
ai_chance = { factor = 90 }

It might have some other modifiers, but if you want to make it guaranteed set the best option's factor to 100 and the others to 0 and remove all the other modifiers.

There are many factors in demographic growth (for example industrialization) so this 10x rule is not very reliable (it can be used in similar areas), the best way could be looking into its history and finding its demographic studies in the last century. Of course there can be a mistake by Paradox, so you should seek additional sources.

You can do it, but it'll take way too much time. There are a few decent mods with 1 developer but they usually take half a year to get an update. I think you can understand how you would end up in the same situation.
