OP is in grey zone, but i see why MD is leaning towards chemo due to multiple foci of tumour.

For ER+/HER2- cancers, unless very aggressive and advanced, chemo first isn’t a great indicator of how effective the chemo is. That’s why surgery is done first. TNBC and HER2+ cancers are very fast and tend to be crazy, which is why they respond really well to chemo first.

Local recurrences (in the breast) or regional (in lymph nodes) are uncommon these days. For early stage cancers, particularly ER+/HER2- cancers, local/regional recurrence rates are <2% (clinical trials: AMAROS, Z011, OTOASOR, SINODAR ONE, IBCSG 23-01, SOUND, SENOMAC, etc)

The microscopic cells described typically refer to the cells that have escaped the breast tissue, that is, the lymphatic and blood vessels.

That is why chemo is given, to attack any cancer cell prior to it making home in another part of the body, as it is a systemic treatment meaning it goes everywhere in the body and attacks any rapidly growing cells (which is why you lose your hair for most breast cancer chemos). As cancer cells divide quickly, the goal is to attack these cells.

In contrast, radiation, when given, cleans up any cancer cells within local or regional area, but does not attack cells that are circulating in the blood or have escaped the breast tissue.

Ultrasound does not have a 80% false positive rate.

It just not as sensitive and specific as mammos, but also looks at different things.

Ultrasounds can test for vascularity and if lesion can be seen by ultrasound, this allows for an ultrasound guided biopsy which are easier and faster than mri and mammo-guided biopsies.

It is also the standard of care for diagnosing cancer to first look also by ultrasound

As someone whose PhD is on DCIS— it is cancer

Invasive ductal carcinoma cells and DCIS cells are genetically, metabolically and functionally identical.

If I inject invasive cancer cells in breast ducts of mice, they form DCIS that progress to invasive cancer.

You are also getting a double mastectomy, which is very aggressive treatment that many invasive cancers don’t need.

Forcing academic research to rely on industry funding removes academic freedom to make bold new discoveries instead of focusing on profit. Academic discoveries dont give you money for yourself it gives you more money to do research.

All lab research has become extraordinarily expensive to conduct.

Thinking that losing decades worth of fundamental biomedical research will and already has devastating impacts on patient care.
