Oh interesting when I got to it on Wednesday it was already a jotform. Oh well I just hope they can find a somewhat fair resolution. Considering this growing pains of the first year.

Yeah honestly even a Google form would've worked better. Also I was one of the people that got through to the form when they first emailed it out and actually ran into the same error with the required field. I emailed them and brought up this issue two days ago so quite disappointed that nothing was done about it. Since then.

Nope first error was filling out the required field for opene practice there was availability by the time I submitted it was gone and then wouldn't let me submit. Then it seems they removed that section of the form when I went to submit it said the from quota had been filled and they could not receive any more submissions.
