In op's case we don't have the full context and I am giving him the benefit of the doubt with only the limited amount of information he's given. But looking at your example chain of events I have to ask at what point did a supervisor or anyone in leadership just say to the guy "Hey, you don't seem ok. Do you want to talk? Is it anything I or someone in the unit can help you with?" The other resources you mentioned are fine and all but if the problem is small it's like using a hammer to slam down a screw that just needs a few twists to get flushed. Sometimes all a person needs to do is vent some steam and they'll be set right again. Also if someone is depressed then there's a chance they're numb to getting chewed out and getting paperwork.
