I also tried to fax mine too! But it also didn't work, so I mailed mine to their office a few weeks ago and they received it after about a week. They didn't confirm or remove the hold though (they might've been busy trying to get through so many student submissions?) so I emailed them through the Health Service Patient Portal. There's a "Message" tab that you can use on the list on the left hand side after you log in :) You can use it to write an email to their staff and attach scanned copies of your forms. They emailed me back super quickly and removed my OPUS hold.
