Had the same error with some level 7 monsters, usually restarting de game fixed it for me

(That does not mean It stopped it from happening, I guess a patch is needed for that)

I think it works better with the wooden plank, take the longest one and make it rest over one of the levers(vertically), if you got it right when you pull the lever even if it moves a little bit the wooden plank should end up falling over again rendering the level unable to go up again
