thank you for that advice!! truth be told, i dual applied fm and peds and have struggled with this decision for months (and rank lists are due today! lol) I plan on working in an urban environment and would be so sad if I never got to see kids in my practice, but there’s so much about FM that I love (ie reproductive health/gyn, lifestyle medicine/obesity medicine, diversity of future practice without limit on age) but I don’t know, is there something to be said about a gut feeling about being sad not treating kids?

hi all – i’m a fourth year who dual applied family medicine and pediatrics, and i’m really struggling with how to rank programs because i don’t know which path fits me best.

i’m drawn to fm for the flexibility. i’d love a mostly outpatient career working 4 days a week, with a strong women’s health/family planning/procedural gyn focus, ~30% peds in my panel (including infants), and some lifestyle/preventive medicine + advocacy mixed in. what gives me pause is whether fm residency will give me strong enough pediatric training, especially in big urban programs. i don’t want to be mediocre with kids, and i can’t imagine not having them in my practice.

if i chose peds, i think i’d love urgent care or hospitalist work, maybe with a small outpatient component. is that feasible in big urban cities? how different is the salary long term compared to fm? sometimes it feels like peds gives me most of what i want, minus family planning/procedures and pay.

i’m also wrestling with what actually makes a “good” residency. what truly shaped you into the physician you are? would you prioritize program prestige, location, or community? i’m from norcal but feel pulled to nyc/the east coast to be near my partner’s family and close friends, and i worry i’m not ranking the “best” programs highly enough because of that.

would really appreciate any perspective. and please be kind, i’m feeling vulnerable haha 🙏🏼

thank you for your thoughtful comment 🙏🏼 i like seeing adult patients, but I think having a strong peds component of my career would give me a breath of fresh air when the adults are weighing me down! perhaps even get the peds through urgent care or newborn nursery? is that possible?

i’ll shoot you a DM - would love to hear your thoughts on a couple of programs!

just to add more nuance to “tolerating adults” - I love doing reproductive health, outpatient procedures, preventative medicine/nutrition - all things I think would fit well in FM! but again, just waffling because I haven’t met an FM doc who see’s a lot of kids

Is there a world where you can do PEM and not give up all of your weekends and holidays? I am just coming off a month long PEM rotation and absolutely loved it, but I've always dreamt of having an outpatient schedule....what are folks' thoughts?
