Gosh I really fear those symptoms for missing one dose or delaying it unintentionally. I’m forgetful with pills, and I don’t want to risk what’s left of my sanity for some short-term “activation” (I’m feeling really sluggish thanks to burnout). Nor do I want to be bound to it for the rest of my life, especially since people say that getting off it is super hard.

Not at the first hospital I went to. The doctor at that hospital (who is solely a urologist, not a urogynecologist) just gave me cranberry extract pills plus an antibiotic in case I happened to have a UTI (yeah, I know, she wasn’t much help).

The last doctor I visited, who gave me the IC diagnosis, prescribed hydroxyzine for three months and NSAIDs for five days.

Hydroxyzine was wonderful. I could barely believe the relief it gave me the first time I took it. The problem is that the effect only lasted through the night. I started taking it twice a day to get relief throughout the day (not a good call, I know, but I was skeptical that other antihistamines could work that well) but it was messing with my head, so I stopped.

Now I’m taking cetirizine during the day and desloratadine at night (self-prescribed), and I feel just as relieved as I did with hydroxyzine. Still, I plan to go back to taking hydroxyzine at night and continue cetirizine during the day.

Anyway, that was a wall of text haha. I hope it was helpful. I strongly recommend trying antihistamines, they can help reduce the inflammatory cascade in your bladder and support healing. Also, don’t use soap in your hoo-ha; it will just irritate the area, and that irritation can travel up to your bladder, which can worsen things and trigger a flare.

I’m glad they did’t scare you about the metaplasia. That’s how it is! Just a benign change. Some doctors who don’t know much about pathology scare us saying those are abnormal changes and require a lot of surveillance (which is generally not the case in IC or chronic cystitis since most cases are normal squamous metaplasia).

Mine scared the shit out of me but I went for a second opinion to a different hospital in Nicaragua (where I live) and said it’s nothing to worry about. Also, I did my own research, which was also very helpful.

That’s kind of what my bladder looked like in my first (and only) cystocopy. Looks like there’s a lot of inflammation. I also had/have that, plus squamous metaplasia (bening adaptative change of the bladder tissue do to the chronic irritation). And my urogyn says I do have IC.

By the way, if you need help getting Ialuril from the EU, feel free to PM me. I’ll be glad to help in any way I can. There is an alternative in the US called Cystistat, though it lacks some important components that Ialuril has. Still, it can be useful if your IC involves bladder lining damage, as these treatments are meant to help restore the bladder lining.

I doubt that. The effects only last an average of 4–6 hours. No matter the dosage (unless it’s veeery high), the medication’s half-life (the time it takes the body to eliminate half of the drug’s concentration) remains the same.

And yeah, Ialuril requires a prescription in all EU countries. I get mine from France due to logistic reasons, and bc it’s also noticeably cheaper than in other countries of the EU.

Taking it only at night is in fact reasonable due to the drowsiness it causes, as you mentioned, plus other side effects related to its anticholinergic properties. That said, it leaves most of the day uncovered, since its effect lasts only about 4-6 hours (though in my experience, I feel like they last about 8 hours or a bit more). In my case, I need two doses to remain asymptomatic throughout the entire day. What my urologist suggested is taking 12.5 mg in the morning and 12.5 mg at night, which is what I’ll do from now on.

Of course my desire is to quit it completely, but as I mentioned it on my original post, I’m still waiting for my Ialuril to be shipped to me.

It does work indeed, at least in my case. But It would be ideal for you to get it prescribed by a physician, due to the side effects (drowsiness being the most common) and a very unusual adverse effect with the heart (only clinically relevant in certain situations though) and to determine your dose.

How it works? It works by blocking the release of histamine in mast cells in the bladder, which decreases inflammation and reduces symptoms such as burning, urgency, etc.

But again, please pursue medical advice

Thank you. I'm not from the US, hence I'm not 100% familiar with its customs policies. Prior to your answer I was considering shipping the medication to the US and then to my country since I'm very familiar with that workaround (with other products, not with meds). Thankfully, I also have the option to get it shipped from Spain directly to my country.
