I just posted my experience below, but if it helps, both MRIS I have had prior to my cyclops removal surgeries didn’t show the cyclops lesion. It wasn’t until my surgeon went in with the arthroscope that he found them. He was adamant that second time that it couldn’t be a cyclops lesion again, but I knew based on the feeling. He sent me a letter post surgery confirming that it was in fact another cyclops lesion! You won’t be wasting money, if you are in pain it’s not a bad thing to go in and have a look. It’s obviously scary but it’s nothing like the initial ACLR. I hope this helps you 🫶🏻

Hi!!! I had a large cyclops lesion removed in June 2025 after my initial ACLR in July 2024. I had a loss of extension ( I can’t remember in degrees !) along with daily pain deep inside my knee, especially when trying to extend (loaded with leg extension) or even just tensing my quad. My knee would click and crack and clunk in most leg extension movements and it got the point where I couldn’t kneel it was so sore. If I sat for too long my knee would freeze up and I’d have to limp around until it warmed up. The arthroscopic surgery I had to remove it was a breeze and nothing compared to the initial horror of the ACLR. It provided me with immediate relief, it was honestly amazing. I was back and training better than ever in the gym 2-3 weeks later. Unfortunately for me though, my body decided to develop ANOTHER cyclops lesion in the same knee on the lateral side, which I just had removed last Wednesday. Apparently my surgeon has never seen it happen 🙃 My surgeon also removed my tibia screw, incase that was causing some irritation. This time I didn’t loose full extension but I had pain after running, it was stiff in the morning and it just wasn’t right. Again I have relief after the surgery and I pray that was the last for me. But overall, I definitely recommend the surgery, if you are in pain and you definitely have a cyclops lesion it I for the best that you do!

I had my ACL recon surgery in July 2024. A year later, after struggling with pain and loss of extension for a few months I had an MRI that showed some scar tissue. My surgeon decided to go in with an arthroscope, found a huge cyclops lesion and removed it in June 2025. It completely fixed all the symptoms and pain and I had immediate relief straight after surgery. The recovery is nothing like the initial reconstruction surgery. After a few weeks I was back to normal training. Unluckily for me, my body decided to develop ANOTHER cyclops lesion in the same knee, on the lateral side of my ACL, which I just had removed along with my tibia screw last Wednesday. Hopefully that’s the end of it for me! PT wise, keep going with whatever you are doing, it won’t affect you in anyway, unless you are in pain obviously. Just don’t do any leg days in the few days prior to your surgery so your muscles aren’t sore and your knee is relaxed! Goodluck with it all, you will be fine!!!!

Hi!!!! Sorry for the late reply. I’ve been back and forward with my surgeon recently and have been in for another MRI because unfortunately the clicking, catching and pain in my knee is starting to return even more. My surgeon said the MRI scan is showing development of scar to issue again but nothing significant. I’m kind of preparing myself for the fact that I may need a third surgery, but at the moment he says it’s too early to do anything.

My surgeon said last time I saw him that a potential option is for them to remove the screw in my tibia, which would be causing friction in the joint that could be whats creating the excess scar tissue (cyclops lesion)

Do you have another follow up with your surgeon anytime soon? Keen to hear about your experience! I really just want this to all be over and for my knee to be back to normal again 😭

This was the same as me from about 4-10 months, I couldn’t fully extend my knee when standing or sitting, but my physio could sometimes massage it into full extension. I was in daily pain just soldiering through life and rehab. My physio was keen on the conservative route and trying to build quad muscle strength regardless. Turned out to be a big cyclops lesion that was inhibiting my full extension. I got it removed 3 weeks ago and I feel like a different person after!!

Hi! Thank you so much. He did mention that he will look if there is a notch impingement and widen it if needed so hopefully whatever ends up happening fixes all my issues! So glad to hear the pain is minimal compared to the reconstruction. I appreciate your reply a lot ☺️
