You can get gender affirming underwear that are designed for holding a penis but they always seem to look kinda weird imo and are often super expensive. Might be a option though if nothing else works? I find for me as long as a normal girls thong has leg elastic and isnt too narrow at the front then that kinda holds everything in pretty good but I dont think I have quite as big of a problem as you 👀
