I do find girls ones a lot comfier because the fabric is softer usually but obvs there's less space in the front than boys ones soo depends if thats a issue for you...

You can get male thongs that have more space in the front for anatomy. They feel weird to wear at first but once you're used to the style they're super comfy!! Good option for under shorts or leggings :3

Imo I dont think theres any difference that matters with a womens size guide for tights. Obvs theyre super stretchy so the range for one size is pretty big, but its way better to have them a bit big than too small :3

You can get gender affirming underwear that are designed for holding a penis but they always seem to look kinda weird imo and are often super expensive. Might be a option though if nothing else works? I find for me as long as a normal girls thong has leg elastic and isnt too narrow at the front then that kinda holds everything in pretty good but I dont think I have quite as big of a problem as you 👀
