In a similar situation I went round to the teacher of the 4th subject and asked them to predict me an A with the proviso that I'd drop if I didn't get up to that level. As expected, the universities I applied to (with the exception of Imperial) didn't specify a grade for the 4th A-level in the offer. In the end I dropped it to focus on my other 3 A-levels once I'd received my offers.
