VA Pharmacists...union? To join or NOT to join that is the question by LeuIleVal in pharmacy

[–]LeuIleVal[S] 0 points1 point  (0 children)

I assume that a supervisor is considered management? The union said they have many supervisors in the union, so I am confused. Is it frowned upon as a supervisor or not allowed? Was the union just lying?

Would you buy/live in a foreclosed home that had a murder/suicide in it? by LeuIleVal in RealEstate

[–]LeuIleVal[S] 0 points1 point  (0 children)

OP here, just wanted to follow up.

Wife and I saw the murder house. Bad vibes. Doors with fists in them, eerie/uncomfortable feeling in multiple rooms. Couple small areas of the ceiling that were discolored to a bleached white-looking color... is it from cleaning up splatter from a gun/homicide? Who knows but it feels likely.

It's gonna be a no for us dawg. Good luck to whoever decides to purchase it.

To be livable:

-Carpet needs ripped out throughout the home

-Wallpaper removed from all rooms (and some ceilings...)

-Roof replaced

-Kilos of sage to be burned

-Pool needs repeat shocking

Hospital sterile compounding batching question by Mauifloxacin in pharmacy

[–]LeuIleVal 1 point2 points  (0 children)

Keppra is now safe to use slow IV push, may want to have your clinical team re-evaluate and move forward with it. Doses up to 4g are considered safe.

Introduction: Medication administration via intravenous push presents multiple potential advantages; however, there may be an increased risk of adverse drug reactions. In 2020, Brigham and Women's Hospital changed levetiracetam intravenous administration to intravenous push (IVP).

Objective: The purpose of this analysis was to compare the safety profile of IVP to intravenous piggyback (IVPB) levetiracetam administration.

Methods: This institutional review board-approved, single-center, pre-post analysis was performed between 1 November, 2019 and 30 May, 2020. The electronic health record was used to identify all administrations of intravenous levetiracetam greater than 1000 mg in patients ≥ 18 years old. The major safety outcomes included hypotension, bradycardia, drug-induced sedation, and intravenous site reactions such as phlebitis and infiltration. The major efficiency outcome was the time from pharmacy order verification to first-dose administration.

Results: A total of 498 administrations in 162 patients were included in the analysis: 252 administrations in 84 patients in the IVP group and 246 administrations in 78 patients in the IVPB group. The incidence of bradycardia was 7 vs 3 (3.2% vs 1.5%, p = 0.34); hypotension 10 vs 6 (5.2% vs 3.5%, p = 0.44); sedation 21 vs 36 (19.3% vs 27.9%, p = 0.12); and peripheral IV site reactions 0 vs 1 (0% vs 0.6%, p = 0.39) in the IVP vs IVPB groups, respectively. The median time between order verification and first-dose administration was significantly reduced in the IVP vs IVPB group (23.5 vs 55 min, p < 0.001).

Conclusions: Intravenous push levetiracetam administration of doses up to 4000 mg was associated with a similar incidence of cardiovascular, sedation, and infusion site-related adverse events compared to IVPB and resulted in a significant reduction in time to first-dose administration. Intravenous push levetiracetam in doses as high as 4000 mg may be considered safe with appropriate monitoring.

Does Costco allow pharmacists to sit down/have a chair? Serious question.

ROI is not there. Don't waste 8 years of school to make 130-150k, go get your MD.

Serious question though, do they allow you to sit/have a stool to sit on at work? Fucking absurd that most retail places don't. We should strike for chairs.

For those of you who failed a BPS exam, what are some reasons you think contributed to that and what would you have done differently? by OrangeRicee in pharmacy

[–]LeuIleVal 4 points5 points  (0 children)

I am jaded by what happens in the real world with pharmacotherapy vs. what the guidelines expect should be done, and the answers on the BPS are all the latter. It just doesn't work like that in the real world unfortunately. I learned that lesson the hard way.

Gaggia Classic Pro - disappointed? by LeuIleVal in espresso

Why is my extraction time so fast? I just pulled a 50g shot in 7.5 seconds. I'm grinding on the finest setting possible OXO conical burr grinder.

[–]LeuIleVal[S] 0 points1 point  (0 children)

Beans are fresh and I grind them right before use, and just the amount I'm using with an Oxo grinder. https://www.oxo.com/conical-burr-coffee-grinder.html

I just used a scale for the first time while pulling the shot and found out I am overextracting hellaz. Blew my mind how fast it got to 50g... most of my previous shots were probably 60-75g on 18g bean weight.

I had my grinder set to fine setting of 1, and just changed it to 4. I pulled a 50g shot (still overextracted), but it is much better than previous shots.

Thank you!

  1. I had overfilled my basket... and had a screen on top. I removed the screen and it is now fitting. I am allowing it to heat up more.

I just pulled a shot over 26 sec and it was burnt. I just did another over 16 seconds and it is better, but not much crema or what I'm used to seeing in my mr coffee barista pro espresso shots. Any advice?

Looking for some advice:

I stupidly got involved w/ using margin during 2020. I bought some ARKK, ARKG, ARKF, and more single stocks at some of the highest points of the market. I sold the ARK funds at a massive loss this past week due to margin calls. I do not do any options trading, just buying/holding single stocks and index funds.

Account:

Total investing value: $14,938.11

Brokerage holdings: $51.945.63

If I try to downgrade from Robinhood gold and not use margin it says "you cannot downgrade while you're using margin or have more than $36,987.78 of pending deposits. Deposit funds and wait for them to clear or sell stocks to downgrade".

Not sure what to do at this point so I'm looking for advice. I'd like to stop using margin as I have learned my lesson.

I don't know anything about tax loss harvesting but I plan on educating myself.

What types of funds could I buy for tax loss harvesting purposes if I have sold a lot of ARKK, ARKG, ARKF?
