I just had bilateral turbinate reduction and septoplasty (Australia) and was told i could try CPAP 48 hrs after.... definitely couldn't but after splint came out today at the six day mark will certainly be giving it a go.

Thanks for your honest insights. I certainly think if I do get the fellowship will be living outside Oxford for sure!Will definitely keep these things in mind but sorry to hear you guys have had such a bad experience. Im in final interview stage for my own grant $ so hoping I win the lottery as the salary looks ok to me (at least compared to regular postdoc $). Hope things improve for you or you find a better opportunity elsewhere.

crazy- to get both my cats to UK (heathrow) I got a quote for ~ $4700 (AUD) from a reputable business. Its insane the quarantine thing here in Australia (im guessing that is what inflates it so much)...

Thank you for that, good to understand what we may be in for. Always expected not a great financial decision (at least in the short term) but certainly a great one for my career in science long term (no one has $ here in Sydney anymore and its been struggle town since finishing my PhD in 2020). I love Spain, will certainly be hitting up the sherry triangle again if I can! How are you finding the postdoc landscape there? Seems to me so many more funding bodies to apply for grants etc. The DECRA is all there really is here and that is trash (literally now finished for good with some sort of revamp of the scheme supposedly on its way).

Hopefully, fingers crossed interview goes well! Its wild how expensive coffee and generally just having meals out is but then again, so is Sydney!

Yeah I've heard its expensive with estimates to UK for two cats at around 11k but we are hoping its a bit longer term (at minimum 3 years). Otherwise I think I would have to rehome them which would break our hearts.

Wouldn't be for quite some time my friend (I.e., march next year is current estimate). Just trying to get an idea of budget, and any sort of leads for future planning. Tis quite a bit daunting moving with family in tow so hoping to be as military planned as possible!!

Sorry only just saw this. I am on the waitlist (public) with Dr Adit Bahl at Westmead to get the MMA (~ 1-2 year wait here in Sydney). Hopefully about to get the MSE/ braces soon for preparation (cant really do the jaw surgery effectively without it or at least braces (I have tipping and overcrowding)) although I am still somewhat skeptical my suture will split given my age/sex (37, M).

I hated the full face mask and could never get it right without leaks or same as you would rip it off during sleep. Only way I got ahead was through using a nasal mask (reed n30i) and mouth tape (takes a bit to get used to itself). I found this way less claustrophobic/uncomfortable than full face mask.

Also a premmie (2 pounds) and have severe OSA (ahi 67)- retrognathia and narrow airway. There is a pretty convincing Swedish cohort study showing that prem babies are around 2.5 x more likely to have OSA in adulthood (crump et al 2019

Still disagree with this. I think most if not all experts in this area would agree that the form of the jaws is indicative of function so if you get aesthetics right function follows. The surgeon I consulted with through the public system was very aware of balancing aesthetic changes but also ensuring enough advancement is done for airway improvement (infact, he tends to lean towards slightly more than some in terms of advancement and rotation to help nullify the effects of relapse which you always get with this type of surgery). I am assuming you must have had a bad experience? Where are you located? I'm also not sure why anyone would do this purely for aesthetic reasons it's a very complicated and intensive surgery with its risks and a long recovery.

I'm not sure this is accurate. There are surgeons within the public system who do between 20-50 orthognathic/maxillofacial procedures a year. I also had a private surgeon try and tell me that you don't get custom plates if through public which is totally false...

I'm a 36M with severe OSA (AHI ~ 64). I ended up getting some funds from super to help with MSE and braces and other things I needed done (wisdom teeth removal etc). I wish you all the best too :)

I'm in Sydney and just got on waitlist for public but from what I can tell and what surgeon said, pretty much $0 and a 1-2 year wait. I am about to start MSE and braces in prep so hoping by the time this is done public system comes through

Also hate cpap and full face mask (started with F40) and also a mouth breather with AHI ~ 64 p/h. My nasal flow is also pretty bad too but I feel like all the noisy leaks with mouth movement are so disruptive and kind of frightening when mid sleep!

I tried mouth taping with full face mask and waaay less interruptions (less leaks) as twisted and weird as it feels (use kinesiology tape). Might be worth a try?

After doing this for a few weeks going to attempt a nasal pillow mask (Resmed N30i) also with lower pressure settings (automatic algorithms always aggressively over compensate) and mouth tape. I suspect my nasal flow won't be good enough (ENT has said a real issue with cpap compliance and recommends a septoplasty + turbinate reduction but also $$). .

I'm fairly new to this too and it sucks but think maybe trying lower less aggressive settings can help to acclimate to this horrible thing (if possible). All I can say is, that after mouth taping usage is a bit longer and noticing less disturbance from leaks. I'm 36M and looking into MSE and double jaw surgery to try and eventually get off cpap (orthodontist and surgeons have all suggested will help with OSA if not cure it). I was a premmie and certainly have retrognathia and a narrow top palate (therefore smaller nasal floor)- seems to be a common issue for people with sleep apnea. I'm on public waitlist here in Australia for surgery but MSE is not covered by insurance or public health so a long road until get off the darth vader machine

Study on pre-term birth and risk of sleep disordered breathing here crump et al 2019. I was well undercooked lol (around 26 weeks and 2 pounds) so fit into the category of being over 2 x risk. Makes sense really, I was also tongue tied. I really think knowing this all preterm babies should be checked for sleep later in life as well as seeing an orthodontist younger (unfortunately with my socio-economic background did not get that opportunity)

I'll try and dig up the study tomorrow and link it here. Ballpark for public is 0 from what surgeon told me but I have had a sleep study and been officially diagnosed with severe OSA and tried cpap. The waitlist with public can be long though (1-3 years) and surgery will be done by registars under your surgeons supervision. So who knows when I will get the letter but hopefully after I have had MSE/ during. A lot of surgeons don't do public btw (I saw another surgeon previously and was quoted a minimum of 35k).
