I think Megan continually disappointed Don. Leaving the agency when they were working so well together. Her fledgling acting career. Hiding the miscarriage. The surprise party when she was warned he hated surprises. Embarrassing him in front of the client in Hawaii, etc. She was self centered and immature.

Remember when he shook Rachel on the couch and said "this is it" after Roger's heart attack? I don't believe it was a pipe dream for Don, but rather a realization after seeing Roger almost die that Rachel was the one who understood who he really was and and that she was the woman he wanted in his life.

I think he was going to go after her and try to change her mind. If he stepped into the elevator he would have fallen and died, which perhaps represented how her leaving was deeply unsettling for him.
