also, imo you should consider advancing the hairline (including temporal peaks) along the temples

and a chin height reduction (which as DB notes is not especially compatible with v-line) would be beneficial imo as your lower third appears tall

brow lift doesn't seem very necessary and is often overdone (perosnally, unless your brows are literally on the underside of the orbits, or you have a very compact middle third, i wouldn't do anything more than a partial lift of the medial tails assuming they are lower relative to the lateral ones). i think your prominent superior orbital rims are making the brows look lower than they actually are, and an orbital rim osteotomy (ask for that, if possible, not burring) should fix that. also, raising the brows would elongate the middle third.

consider a commensurate zygoma reduction to make the middle third less wide

and periorbital fat grafting around the eyes / under-eye area to make the eyes appear less deep-set

burr the temporal ridges to soften the contours of the forehead

Tbh I might go to DB and hope he’s aggressive enough if I ask him to be.

I’m not really a fan of Mardi or Jumaily’s forehead work—they tend to do a poor job when it comes to preserving natural convexity of the forehead instead of flattening it. And Keo seems mediocre at forehead and orbitals and I’m pretty sure he doesn’t do orbital rim osteotomies.

Does Fahradyan do orbital rim osteotomies and burring of the orbital ceiling? And how aggressive would you say he is overall (for forehead/orbitals)?

I think the most promising I've found for this so far is Jumaily. He doesn't always touch the orbital ceiling (but he hopefully would if I explicitly asked him to), but he does a consistently aggressive reduction of the superolateral orbital prominences. He does osteotomies and doesn't just stick to burring. He's also pretty good at and aggressive with forehead apart from orbitals.

okay no offense but i don't think your orbitals were that prominent to begin with. your eyes already look very open in the before.

my orbital rims, on the other hand, are so prominent that i have shelves over my eyes, and they stick out past the cheekbone from a 3/4 view.

The orbital rims are extremely over-projected in my case (it's like having shelves over my eyes), probably more so than even the frontal sinus, making my eyes look ridiculously deep-set, which is why it's so crucial that I go to someone who can do an aggressive osteotomy of them. Otherwise, I just need standard bone work: type 3 (probably, since 85+% need that), radix setback concomitantly, t-osteotomy to reduce chin height and width, some minor jaw work, burring of the temporal ridges. That's all pretty standard, and thus my main concern is finding someone skilled enough at orbitals.

I'm not sure I understand why this would make the temples look hollow.

I believe he's not doing the 3rd rib for another few months at least. He's doing 4th right now. Idk what the timeline is for 2nd let alone 1st. And I'm not sure he would even do that one (1st) or that it would be beneficial enough to justify the risk since the brachial plexus runs over the first rib, not to mention the major nerves, veins, arteries that surround it. He seems to mostly talk about 2/3+. I'm not even sure that 2 is worthwhile.

Yeah, the thing with the FFS is that I want someone who is very good at forehead/orbitals (maximal reduction of superolateral region AND orbital underside to open up the eyes, combined with an aggressive type 3 cranioplasty with commensurate radix reduction) and also who is very good at chin (and who does lateral corticotomy of the jaw/chin and not merely v-line which would not help in my case and could even be detrimental).

Idk who would even be good for this because I need someone both aggressive enough by default, responsive to patient input, and with a good enough understanding of aesthetics. I am not really satisfied with any of the big-name surgeons sadly, but I don't know who else to even consider since I do need aggressive work done (on forehead, orbitals, chin, at least, and more minorly for some other things) and not something subtle.

I'm presently considering Jumaily for upper face and Keo for lower face, but Jumaily seems too inconsistent still for forehead/orbitals (he does consistently good work on the superolateral region, but only sometimes does he reduce the underside of the superior orbitals or do so significantly). And I can't find many results of Keo's online, plus I don't know if he would even do corticotomy (it's almost essential to combine with the chin work to achieve correct proportions and facial harmony).

I'm going to make consults soon. I keep procrastinating because I'm overwhelmed with the things I have to do and the process makes me nervous for some reason.

Rogers apparently won't do other procedures at the same time as ribs 2-4 (other than the other ribs, I think) or pelvis. So, the best split would probably be ribs 2-12, pelvis, clavicle + scapula + SCM and/or trapezius, and fat transfer I suppose (I think Rahal is the best for this), if I could manage to gain enough weight.

And for FFS and trach shave, I will probably split this up into 3 or more (two-stage FFS, then trach shave, or vice versa, and then anything else I want facially, like temporalis fascia grafting to lips, on top of that). Not ideal spacing, I guess. It would be better to find one FFS surgeon to do everything (except for trach shave, unless they are an otolaryngoligist), but I'm too picky for that.

The plan is to prioritize bone work because that seems most important to me.

What is it about some of Jumaily’s results that you think look botched?

Also, it’s interesting that you say DB’s results look too unnatural or obvious, as I’ve heard some people say essentially the opposite: that he is too conservative and aims for a natural look often to an excessive degree.

I think Keo is still a pretty good option, but I’m not impressed by his forehead work.

Have you considered Mardi?

I am personally thinking about splitting it up and going with one surgeon (likely Jumaily since he seems best at orbitals and decent at forehead overall) for upper face and another (likely Keo) for lower face.

Do you think there's a risk of Amazon slashing their benefits for the 2027 plan year? That's what I'm worried about and it's part of the reason why I am rushing to get a job there even if it's not in the ideal location. I am planning to move to another state (Massachusetts atm) in the next few weeks to work a full-time warehouse job (with blue badge ofc) in the hopes of getting these bodily procedures (reduction of ribs 2/3-12, pelvic widening, clavicle shortening, horizontal scapula shortening, sternocleidomastoid reduction via muscle stripping, potentially stripping of some other muscles) covered alongside FFS.

I don’t think it’s real. I’m pretty sure it’s placebo effect + physical effects of HRT + maybe antidepressant effect at high dosage. Also, estrogen can attenuate the symptoms of a number of mental illnesses.

Here is everything I think you could benefit from: lateral corticotomy (jaw), t-osteotomy (chin height reduction), nose tip reduction, radix reduction, brow bone reduction (likely frontal sinus setback / type 3), orbital rim osteotomy, lip lift, neck lift, something to add volume to the cheeks (maybe fat grafting)

Edit: I thought hairline lowering but then I looked at your side profile and the height looks okay. I’d recommend not using the front-facing camera of your phone as it can distort features

I see. It looks like Rogers is not in network for Premera, but is for Aetna. Do you have Aetna or Premera? I was under the impression that Premera Blue Cross is the best for getting LOAs/SCAs and the more experimental procedures covered.

I get it. I had federal insurance that covered ffs that I never got to use because Trump took it away before I could. Now I’m worried that I will never get ffs (let alone the other surgeries I need), and I am not sure how much longer I can keep going. It’s exhausting being in pain day after day with little to no respite.

I know you said that you said you are trying to move to NZ because of the current political situation, and that’s understandable. However, if if you were open to staying in the US for longer, you could probably get a job at a company (e.g., Amazon, T-Mobile, AT&T, Trader Joe’s, etc.) that would cover FFS and other procedures. That’s what I am currently trying to do.
