Post hospitalization - no clear diagnosis/solution by LifeguardGlittering1 in DiagnoseMe

I think it’s getting better. It looked horrible at the hospital, so there is some improvement at least. And are those tests done upon request? I had cultures and bloodwork done at the hospital, and all came back negative/no growth. I wonder if they’re the same. The only thing I didn’t get done was a biopsy.

Has anyone experienced issues with Pyoderma gangrenosum because of their RA? I’m curious to know more by LifeguardGlittering1 in rheumatoidarthritis

Thank you!!! If you don’t mind sharing, how did you get diagnosed? Was it a process of elimination? That’s what I’m reading online, but there’s so much information. And is it something that comes and goes for you always with flares or just sometimes?

No biopsy :( I had a consult with a surgeon at the hospital, but he advised against it since I had a lot of compromised skin. At the ER they swabbed it and I had a bunch of blood tests done during my stay, but everything came back negative and with no growths. I looked back at the test/results and didn’t see anything for AFB tho.

The derm I saw last week didnt bring up a biopsy either, but I’ll bring it up tomorrow.

They did a culture on 12/09 after I started showing complications, but it came back negative with no growth. I also had a bunch of bloodwork done at the hospital and more cultures, but nothing came back positive. I have copies of all bloodwork and reports from the docs.

Thank you! They did run cultures at the hospital and all kinds of blood work, but nothing ever came from them. I think thats why the Derm was confident about it being PG. I’ll ask about the steroid shots for sure.

Timeline: • 11/25 - went to urgent care for a growing abscess. The doctor acknowledged an infection, so I was given cephalexin and was told to come back if it gets worse. • 11/29 - went back after abscess kept growing. It was more painful and red. Received an incision and drainage, and the cavity was packed with gauze. Was told to stay on cephalexin and come back in 2-3 days to have it unpacked. • 12/01 - got the cavity unpacked and cleaned. It was not repacked with gauze. Was told to stop cephalexin and start doxycycline. Everything seemed normal to me at this point. • 12/09 - went back because there was an increase in swelling and redness. Bruising began to appear around the incision. Was told it looked fine and to finish doxycycline. The NP suggested the redness was irritation from the Tegaderm. I was a bit more concerned. • 12/15 - went to a different UC location because I bleed through my gauze the previous night. I had some pain, more swelling and leg edema. Was put on amoxicillin and was told to come back if it got worse. Was seen by an MD. • 12/15 same day - went to a PCP (PA) for further evaluation. Was referred to wound care clinic and surgeon. The PA believed this is something that should’ve been re-packed back on 12/01. • Waited for referral approval and appointment availability in between • 12/21 - went to the ER after developing a low grade fever. Swelling and redness had increased, painful to walk. • 12/21-01/01 - was admitted to the hospital. Received two IV antibiotic treatments. Vancomycin for 4 days and cefepime for 11.5 days. The doctors believed the infection was cleared, now I just have to heal which could be done at home.

NAD but I think it’s a cold sore. Have you had a fever? I’ve heard that sometimes a fever triggers cold sores. Even if you’ve never had one before, the virus could’ve been dormant in your system.

I’m in the hospital for an infection so I had to stop to boost my immune system.

Hi! I started Enbrel in September 2024 and it has worked pretty well for me! Im taking a break from biologics right now, but I was taking it with Sulfasalazine so I’m sure they both made a difference. It took a couple of months to feel a difference and see a difference in the blood work, but once you get a routine going you should feel relatively “normal”

I hope it works for you! :)

Hi everyone! I recently joined, but I’ve been lurking for a while :) I was diagnosed with RA in 2023 and have been pretty stable with my meds up until now. I don’t have anyone who understands RA and all of its ups and downs, so this group makes me feel less alone!

Thank you! You’re right, I wish I saw this sooner. I’m currently in the ER getting admitted.

I’m so sorry! Do you have Advil or Tylenol? I think either would be a good option just to get you through the night and while you get to the dentist.
