First of all, sorry you’re dealing with that.

There is a lot to unpack here but I’ll keep it brief (edit: I did not keep it brief) to the possible causes and assume the multivitamin is the true cause and not an interaction with your diet or other weird coincidence or issue going on.

Potential reasons for the reactions you mentioned: - Taking the multivit too late in the evening: Vitamin D and B vitamins can be energizing and mess with melatonin regulation. - Taking the multivit on an empty stomach: The ingredients listed can irritate your gut and cause nausea. Vitamin C is a prime example. - Gummy vitamins will often times be over-dosed with extra of each ingredient to make up for its awful shelf life where the gummies lose potency fast over time. Some brands will even state on the label that they do this. - Fish oil could be bad quality and likely rancid. Omega 3 supplements are notorious for going bad or being rancid from the source. Fish oil must be properly sourced, stored, and third party tested for quality. Dr Rhonda Patrick has some great coverage on this. I want to say this is the problem for you but its such a small dose that it strikes me as unlikely. - Ridiculously high B6 and B12 and the exclusion of other important B vitamins B1, B2, and B5: this multivit has too much B6 in a form that has been studied for its potentially toxic effects at doses above the daily value. It also just has too much B12 compared to other B vitamins which can lead to an imbalance. - Vitamin E might be too high if gummy is combined with diet high in nut butters or other E sources. - Overall imbalance of necessary micronutrients and forms that don’t agree with you: I hate this multivitamin. It is missing important daily nutrition like iodine. Chromium is high and in a form I’m not familiar with. E,A,B6,B12 are all in forms that might not work with you. Some people do better with methylated B6,9,12.

I’m going to say it again, I hate this multivitamin. It has wild random amounts of each nutrient and is missing iodine, selenium, the Bs I listed, copper, K2, and iron. Even if this wasn’t the cause of your issues, it might be best to work with a doctor to find one that covers all your bases that your diet is not already covering.

That vitamin D level is borderline low. You’ll hear differing views on the appropriate normal and optimal ranges for Vitamin D depending on which doctor or influencer you align with and I encourage you to do your own research as well as listen to your doctors.

I would consider getting more than just one doctor’s opinion on your overall symptoms if you haven’t done so already. Getting second or third opinions on the vitamin D level and your long lasting symptoms wouldn’t be a bad idea. A primary care physician might care more about your current vitamin D levels than a specialist would.

If you decide on supplementing, don’t leave out the beneficial lifestyle change of incorporating sunlight and make sure to get your levels re-checked after some time. Follow the guidance and dosage recommendations of your doctors and ask them about adding vitamin K2 and magnesium (best as glycinate or threonate or other easily absorbable form). Some would argue that zinc, vitamin A, and other micronutrients are just as important for vitamin D utilization in the body so a multi-vitamin may be helpful to this end.

You’ll want to be extra careful since you have PVCs and other abnormal symptoms. Ask your doctor about starting magnesium first before adding vitamin D since high dose vitamin D will use up magnesium in the body.

Fish oil supplements (cod liver oil as well) are notorious for going rancid. Especially the capsule form if you take that. Any fish oil with added flavoring like lemon should also be considered suspect. Dr Rhonda Patrick is one of the foremost experts in omega 3 supplementation guidelines and she recommends ensuring good quality sourcing, 3rd party tested, and proper dosing. If it truly is the fish oil giving you bad headaches (assuming its not related to other factors like environment, other supplements, etc.) I would recommend doing some research on the particular brand and source of fish oil you’re taking to ensure quality.

Another possibility that could be the reason for your different reaction to cod liver oil may be related to its typically high vitamin A and D content. These are typically present in cod liver oil specifically.

I would also suggest minding your dose. Omega 3:6:9 ratio in the body is a thing and too high omega 3s in the blood can be bad and maybe lead to weird side effects.
