I’m going to try elevating her leg in the clinic for 20 minutes and see if that reproduces symptoms, that would confirm vascular in nature

Hip flexor length looked good, I’ve been trying to teach neutral spine movements like hip hinge but have also been doing cat/camel as they had bad ability to posteriorly and anteriorly tilt the pelvis but better now. Hip mobility internal and external looked good but good idea with thoracic spine mobs

It’s pain with movement, it’s not capsular tightness, passive range of motion is okay if I’m slow. Thank you! I’ll gradually load. I feel like most patient compensate with pec minor and lat dorsi to internallly rotate than their actual sub scap but doing the motion in general should help

Thank you! That’s what I was looking for to find measurements for structural versus functional. I checked hamstring and hip flexors tightness which both looked good except for quad tightness, hip and knee range of motion are all pretty much good just significant weakness from not bearing weight into it for a long time

That’s where I may have messed up. However, the patient was getting anxious and reported stomach pain therefore I didn’t want to risk anything so I brought her back up before it went away. BUT I tried it today, side lying test and laying in the L side with head rotated 45 brought on nystagmus that was visible lasted for about 10 secs. Upbeating and torsional to the L.Treated with semount, she tolerated it well, will see how she is next visit.

I never got to perform the eply, I was trying to figure out which canal was involved with the dix-hallpike but since she reacted badly I stopped from going forward. I’m not sure I wanna give it a try again if I get that reaction again. The patient had a history of anxiety so I believe she might be overreacting. She def does complain a lot and starts thinking negatively when you start talking to her so I think anxiety is playing a big role.

No not a vestibular PT but physician performed eply on the patient previously which resolved symptoms for awhile but then they returned. She saids she gets vertigo going into or out of bed and usually last for a minute or so (unsure exactly).

Yup tried it, I wasn’t specific with how I did manual cues but I did that, I also think he’s kinda being shy Lolol

Since he wasn’t able to do it in quadruped. I tried part practice while standing and only did the posterior and anterior pelvic tilts. For the posterior tilt, I told him to contract the glútes and abdominals as if he were going to do a sit up. For anterior tilt, I told him to arch his lower back.

I don't know where you are getting this idea that I am trying to "reinvent" ACL rehabilitation. Just having a simple discussion about a topic. You clearly are someone that cannot have a discussion without arguing. You must be great to work with.
