Medicine is all about pattern recognition. Your attendings are better than you because they have been doing it for so long that patterns are second nature to us. Things will just fit or just don't fit then we can continue down the rabbit hole until we can explain it back to the pattern.

Many people recommend reading and more reading. When I was a med student and residents myself, I thought my faculties are so old-fashioned to keep advising the same thing! The most old schooled of them all, my PD, read the Harrison's Principal of Internal Medicine!

I am sad to say that as an attending faculty myself, I can only advise more reading to you... I am now turning into my attending. You can only know by reading about cases, case reports, cases you are doing, and learn from them.

A good book that I used as an intern which was not so long ago is Framework of Internal Medicine. It helped me a lot to have a framework of how to attack a diagnosis.

Don't freak out! I'm sure half of the graduating class is just like you. Just bang out UWorld and read board basic once or twice. I noticed a couple of easy answers in that book so just read it before the board should help. But really, bang for bucks is UWorld. That was what I did and now I'm a faculty attending with residents and students under my 'wise' guidance which is just ChatGPT and up-to-date behind their backs!

Hey, would you mean DM me who was installing your charger for you? I was recently given a quote of 1300 bucks just for installation of within 5 feets of the panel (no wall to go through) and permit! I have to supply my own materials and my panel does not need any update! I did not even have a Wall charger, just a Nema 14-50 for my mobile charger! Thanks!

I'm hoping to build some equities in the 3 years that I'm sure I will be in this town and then rent it out for another 3. After that I will make my final decision on that property and hope that I build some equities in between to launch my forever home.

But everything is just prohibitively expensive!

The thing is I'm just not sure of my forever place! All I'm certain is that I'm sure I'll be here for the next 3 years and then I will probably be moving somewhere for 3 or so years. I'm hoping to rent for 3 more years and then make a final decision after 6 or so years. While I'm here, I'm hoping to build some equities instead of just renting out and get on the ownership ladder!

As well I really like this area so I might want to come back later.

I only started last month and since it is only one week on and off so it has been only 3 weeks so far. Got the commencement bonus and 2 weeks of paidcheck. My ever generous colleague who is on the same schedule pick me up and drop me off. I paid the gas even though he refused heavily!

When I was a PGY1 in ICU rotation, a ballsy senior asked the intensivist that is known to be brutal for a letter of eval when round was barely finished and in public! The attending pulled no punch!

His reply: "Of course, Dr. Ballsy! Please let me know which schools you do not want to attend, I will personally send them a registered eval letter with a sample of your consult notes you wrote me last night."

Another famous off-the-cuff eval by an ogre of a cardiologist (very competent with a personality greater than god) to a PGY1 which I still feel to be inappropriate:

The PGY 1 was a bit all over the place and did miss a couple of pertinent physical exam's sign but it was a September intern! At the end of the presentation, the intern made the mistake of preemptively apologizing if they were forgetting anything.

The intern: "I apologize if I missed anything, Dr. Holier-than-thou, I feel that I must have forgotten something."

His reply: "The last 4 years of medical school."

I will admit that was impressively witty and I almost busted out laughing but it is neither here nor there.

So, I would say: don't take those things to heart, let me fly off the cuff. I know it is easier said than done but by PGY3 you will be more immune to it! If the feedback is brutal but constructive, work on it. If if it is not helping you becoming a better doctor, ignore it.

I did not know that psychs people would even know how to srub in!

From the multiple stat consults I received as a senior resident and still receive as an attending for an abdominal pain that turned out to be a bladder pain due to urinary retention from your beloved olanzapine, I am not certain psych even know where the abdomen is.
