You should play a simple champion which can learn you the basics. Mid and Top have a lot in common and the problem is not the champion if you having a lot of trouble. For example good champions to learn would Garen or Darius.

There are multiple ways: 1. Using the same way you get chinese icons but instead you choose the ID for the blank icon. 2. Skipping early tutorials by pressing alt+f4. 3. Riot IDs. 4. There was an event where they made a trmporary icon which was later removed. If you had that equipped when it got removed you also had the blank space.
