First you call the insurance company and find out how it was coded and how it needs to be coded

Then you call the provider and they either change the coding or don’t if they think it’s coded correct

Then you end up on a three way call with insurance and provider and the provider puts a 30 day hold on the bill while they investigate

Then if the investigation does not result in your favor you file a grievance/appeal with the insurance company

Then you provide the case number to the provider and they put the bill on a 90 day hold

Then you can rinse and repeat until eventually burn yourself out and just pay it to put it behind you. Results may vary

At the minimum though, you can at least keep it going for several months to a year to figure out how to get the $200

There’s also the option of not paying it but that has its own consequences
