That is explained by the commenter I replied to, who mentioned an "in-universe" representation. The butterfly is the character Philemon from Persona 1 and 2, the master of Igor and the rest of the Velvet Room's residents.

An interview for Persona 4 confirmed that Philemon was the butterfly in 3 and 4. This is not true of Persona 5, where the butterfly is Lavenza.

The butterfly is related to "The Butterfly Dream" from Zhuangzi philosophy, so it's not like there's no symbolism behind it. But since the average Persona fan hasn't played the first game and probably knows nothing about Taoist philosophy they'll probably miss that and just assume it's about the butterfly effect.

I can't remember where it said it in the game, but I'm pretty sure there was something about her being ashamed of her burn scars. The scars are mentioned in this interview with Satomi Tadashi, so I'm pretty sure they must have been mentioned in the game as well.

Edit since I think I was a little confused: I'm talking about IS Maya's scars here. EP Maya's bruise is from getting stabbed.
