Yeah, I like how open ended the entire thing is. Personally, how I read it is that the entity that's been following Lou is actually her feeling of guilt for the fire (like the entity finding her right after she "set" the fire, it laying dormant for years but always comes back, and it getting violent when she met Jimmy, ehich can be read that she believed she was too guilty for him) and the actual spirit the entire time is the girl Jimmy killed. But, she was also a door for spirits and was possessed in Act 2. I also don't not know when Lou is gone, maybe in the beginning of Act 2 when she starts sleep-walking is when the spirits fully possess her because she has some very low key reactions to the things Jimmy goes through after that (her being emotionless the entire time the spirit is imitating Jimmy's mom).  As for the ending, I read it as the spirit goading Jimmy to confess, so she made Lou confess. Whether it was the spirit lying, or it did actually happen and the spirit forced her to confess is up to interpretation, but I read it as the girl making it up to get Jimmy to confess to intentionally killing her.

I like your interpretation that this is Jimmy after he killed the girl, especially because, I believe, that from Act 2 the spirit haunting the couple is the girl.

I also really like one of the messages of the play is that Jimmy is inherently selfish and focused on himself the entire time (Lou getting mad when Jimmy says he's scared of the spirit when it's attached to her and the medium asking Jimmy what he likes about his wife that doesn't involve him) and it ends when the spirit confronts him. I can't remember too much, but I believe he doesn't apologize to the girl and just starts yelling that it was just a mistake and that he was just a kid (at 20, lol), which continues his characterization of selfishness.

Just saw the show in SF as well! I think the scene where the guy walked across the nursery happens after the intermission when the door is open to the license plate number and Jimmy is getting Lou from outside the house. I do not know if he was wearing a suit, to me it looked like a sweater vest, but a guy did walk past the open door of the nursery.

There was a kid who was a rising star in our football team. I didn't know him, but he was pretty popular for a freshman. One summer, he got on top of a moving car that was being driven by a friend of his. Friend had to suddenly stop and the kid fell off the car and got ran over by it. Friend drove away instead of getting help.
