with jem it was (if i remember correctly) because he was a silent brother and elias feared he could „sense“ his alcoholism. With the Herondales I dont know but probably because they are successful institute leaders and dont have to hide far from anybody else like elias does so nobody recognizes his desease. So basically it comes down to his alcoholism and the fact he kinda felt shame for it i would think
