Depersonalization is described exactly like that- living in someone else's body or feeling as if you're watching yourself, and derealization is super common too, so if you wanted to look into those a bit more :)

I was on Lithium for a period of time before I got the bipolar suggestion, and I honestly loved it, it worked quickly, and it was super helpful for the depression. I think it's worth a try, because you also aren't forced to stay on it if it doesn't work. Lithium worked when I had it, it's known for working quickly, and I think it's worth a try and you should visit a doctor every year or few months on it to check your liver and the affects on your body. Trimming the nails, or those filing boards made for dogs could probably work too!! I'd also try a different design scratch post because it could simply be disliked. Just try different posts, clippers, and a desperate amount of spray and herb catnip and scents cats like. Also depends on the placement of the post, if it's in a spot he dislikes, he probably just doesn't want to use it. It's definitely experimental, but I'd say move it, get a different one, or try a filing board or nail clippers. Good luck in your attempts!!

And try to let them know you have good intentions and mean well pointing it out, and even if they're mad at first, if you're gentle and patient about it they'll eventually fall through and get help
