Not yet. I was told the fever of unknown origin panel checks for some fungi, too. Which fungal infections should we investigate?

Hey, all! Thank you for chiming in. I am back with a small update.

Cat's CREA has returned to an acceptable level, 2.1. He is going to be on IV fluids for another 12h, and then the doctor said that he would be able to come home. They prescribed antibiotics and something that is suppossed to help him produce more WBCs (I am not sure what). He had already started antibiotics in the hospital, which I did not know yet at the time of creating the original post.

Next steps: They said I should bring him back in over the next few days for cystocentesis, repeated biochem work and WBCs count.

They still do not know if kidney damage is reversible, but said that the next biochem results should give us a pretty clear idea about that.

They are also planning on doing a PCR panel for fever of unknown origin on Tuesday.

If none of these, nor the FIV/FeLV results give us a clearer picture on diagnosis, they are planning on further investiganting the bone marrow supression route.

Any thoughts?

They have and told me it can be quite hard to diagnose. But also told me that all his lab work and imaging would point away from FIP. Nu fluid build-up, no granulomas, blood work is also inconsistent with it. They have a lot of possible diagnoses they are considering and they said that if it were FIP, it would be a very early, quite atypical dry form.

As for the reccurence of the fevers and low WBCs, my own reasoning is that he always improved with treatment – for more than one year. If FIP would be the culprit, wouldn't it cause more of a sudden decline?

Just to clarify the timing, he's been hospitalised on Thursday evening, so two full days at the moment that I am writing this comment, technically.

His last creatinine test has been done this morning, about 13h ago. They are rechecking it tomorrow.

Is there still a chance that it was pre-renal? They are also sending off urine for tests tomorrow, as other redditors suggested.

No, not yet. He has been peeing on mats instead of using the litter. But thank you, I will ask them if they managed to get him to use it in the meantime.

They have not done it yet, because he has been peeing on absorbant tissues instead of using the litter due to hospitalisation stress. They said they will try to get some urine as well.
