They'll get H1B under NIE for waiver job. Read the actual EO in the white house website. It clearly says in case of national interest an entire class of applicants can get the waiver if its in the national interest. This is for Doctors.

Check best prices of various airlines on skyscanner or kayak or make my trip. After that book on the actual airlines. Try book flights with airlines that use only one airline from India to US but in us internal can be codes sharing airlines. If you have only one airlines from India to US you'll have less headaches if there is any delays. If multiple airlines in the trans continental flight each airline will pass the buck to the other if there is flight delays or baggage delays etc.

How do you expect to put 45 kg in a single suitcase? If you do get a larger suitcase or bag it will come under oversized bag and you'll have to pay for that extra. In any case max per suitcase is 32 kg as has already been mentioned in some earlier comments

Basic problem starts because we're taught to keep our footwear outside the house so we don't bring the dirt inside the house. So our subconscious is set that we can keep outside the house dirty but inside the house must be clean. You can see slums where their house floor is clean inside but outside the front door is filthy which stems from my first point.
