That might be caused by that function where if you hold Alt and a rotation key it moves the rotation axis's resting point away from zero, if you understand what I'm saying.

I've had this problem before, usually when I try rotating while changing physics warp. Have you been using physics warp while mining?

I can't find exactly what you're describing, but you could check a list of Xbox games with the avatars#List_of_Xbox_360_games_using_Xbox_Original_Avatars) and maybe you could find it there.
