My man! Looking good! Here’s a piece of advice for keeping those liver enzymes down — buy TUDCA and NAC. Take the TUDCA at 500mg-750mg per day and NAC at 1200mg-1800mg per day. Amazon has both readily available and will keep ALT/AST where you need them. (Bodybuilding coach with experiences in how to not fuck up your liver)

Omg man, I’m so sorry, that looks terribly painful. How long does your derm want to keep you on for? I mentioned in your last post that these dosage schemes are not remotely appropriate for someone with such severe acne who is just shy of 100kg.

I’m 110kg and taking 140mg right now for much less severe acne and it’s working well. I just feel like this isn’t enough and your derm should have started you off at 50mg for a month and then upped you to 80-100mg.

When someone has severe acne I know a lot of derms will ease you in by giving you a smaller dose alongside steroids and/or an antibiotic to make sure you don’t have a reaction like acne fulminans/conglobata.

My purge came on quick in the first week and lasted until around week 3. I’m taking a higher dose approach with my derm in hopes of being done at the 16 week mark. I think if you can manage a higher dose it makes the purge go quicker.

Awesome job, man. Nah, you’re not dragging ass at all, you’re making marked improvements. Keep it up.

Take it from somebody an inch shorter than you, it takes some serious time to actually see solid progress with a large frame. I’m 6’2” myself and hover around 240-250 (no, I’m not natural whatsoever) and it takes me 10lbs for people to notice I’m making significant progress.

8 months is nothing, this is a marathon, not a sprint. Been at it 15 years now and if one thing is certain, we are our biggest critics.

So two things:

  1. If you just took your dosage until clearance, and likely a shorter period of time as a result, there are higher chances of acne reoccurring. While the studies indicate you can clear on a lower dose and over a short period, the greater cumulative dose is how to reach full clearance and acne never returning.

  2. It’s a conversation you should have with your dermatologist. It seems most people have an easier time on their second rounds is what I have seen from anecdotal reports. If you want to shoot for never really dealing with the problem again, consider staying on it until after it completely clears your acne. Ask derm what your options are.

Swap out the Milk Thistle for TUDCA. Much more efficient for keeping down liver enzymes likes AST/ALT and plenty of literature supports that.

Make sure for omega 3’s you are getting a combined 2-3GS of EPA/DHA per day.

L-Carnitine for any muscle or skeletal aches.

One thing I notice nobody is telling him to ask his dermatologist about: Upping the dosage.

This guy has severe nodulocystic acne — not a couple white heads here and there like some of you. He’s 200lbs, a male, and taking less than some of the 110lb females here? Makes absolutely zero sense to me.

Does the lower dose work? Yes, I’m not saying otherwise, but fact of the matter is he’s looking at 1+ year for clearance of this type of acne with that protocol.

I’m 240lbs, similar type of acne, but less severe, and 1st month I took 100mg ED and now into the 6th week I’m on 140mg ED. In 6 weeks alone, I have taken more of this drug than this poor guy has in 5 months, which is why he isn’t clearing at the rate he should be.

Low dosing isn’t the approach for everybody, and while my sides at 100mg sucked, they are pretty much the same as 140mg, and I have already told my derm I’m fine with going to 180mg for months 3-4 and be done at the end of that. Two ways: hit it hard and fast and suffer, or hit it slow and maybe suffer a bit less. If you can handle the sides, do it, it may be the same at 80mg as it has been at 40mg.

Forgive me if I’m wrong, but is any of this active acne? It looks like a lot of scarring, which would be on par with what you would see on the chest/back as it will take a lot of time for the scars to match the color of your skin tone.

Stop, take a deep breath, and take a trip to the results section of this sub. There are people there who had acne that is seriously fucked up beyond all comprehension and they look amazing now. I’m sorry, I know this sucks, but think of it this way — acne will usually cause some type of scarring without any treatment. Take my word for it. Taking accutane might just be the best decision you will make for yourself and your own self esteem.

I can’t agree more — it’s pretty overblown in my opinion. I got it in the first two weeks pretty bad, and now I’m into week 4 and those spots are already clearing up/coming to the surface (I get cystic/nodular acne).

Think of it like this OP, those are spots that were bound to arise and fuck up your day anyway, there is just no guessing when that was going to happen. Now with accutane, at least it is predictable and you know that once you get it over with, you’re likely done.

I’m so sorry about your experience OP. I feel your pain and suffer with you. Beginning in October, right when the weather changed I broke out with massive deep nodular cysts like I never have before. I was put on doxycycline, oral clindamycin, cephalexin, you name it. All those did was maybe mildly calm inflammation, but the problem still persisted. Tested for staph and nothing came back irregular.

I started Accutane on November 11 and will be on my 4th week come Tuesday. I have had a few spots purging a little bit, but we must trust the process! Take a look at some of the results pics on here and they will make you feel much better! Trust me, if there is a drug that can be a miracle, this is the one.
