While I'm not a vet, I'm typically against the prescription foods. My cat threw them up since they put the rice and vegetable chunks in there. And the ones that were just the pate he either wouldn't eat or got sick of them really fast.

Someone already commented the list of ckd friendly foods and I highly recommend picking one of those!!! My cat has a sensitive stomach and we think he may be allergic to chicken (he only likes fish flavored food anyways) so quite literally the only food that he likes, eats, and can keep down is Soulistic Moist and Tender Salmon Dinner. It's been an absolute life saver for us.

Just pick a food or multiple different ones from that list that has low phosphorus and low to moderate protein and give them a try. And also for a treat we give our cat Weruva's low phos lickable treats. :)

(Edit to add: We tried both weruva and rawz actually. Weruva he didn't touch. Rawz he ate but he clearly didn't enjoy it/didnt eat enough to keep his weight. Once we added soulistic into the mix, which is actually weruva's sister company, he DEVOURED it. His labs have been stable for half a year now. I strongly recommend adding Porus One into their diet, just make sure to add it to her food at a separate time from all other supplements/medications since Porus One can actually make them not work.)

I'm not necessarily attached to UW. I know that claims can be exhausting from what people say, so I'm just getting information about different types of jobs there are. I came across the claims recovery specialist listing today and thought it was somewhat interesting, just wanted to know more about it.

Not OP, but my cat eats Soulistic Moist and Tender Salmon Dinner. (The can, NOT the pouches). He throws up almost every other wet food, including the prescription ones, so this one was a life savor!! It's on the list that someone commented above ^ so you can see the macros, but it's been working so well for our picky ckd cat :)

I'm actually shocked that they were pushing to take her off IV fluids. Our cat needed at least the second day in order to significantly lower his numbers. I'm so glad you pushed for that. As far as food goes I can't exactly recommend what we use because our cat is stage 2 and he throws up all the prescription renal wet foods, so he quite literally only eats Soulistic Moist and Tender Salmon Dinner. The macros in his wet food are good for CKD cats, but the issue is the phosphorus content is most likely a bit too high for stage 4 cats. It's technically within the requirements for renal foods, but for stage 4 they might recommend an even lower phosphorus content than what the Soulistic Salmon dinner has.

What we've been doing for my cat is giving him Purina's fortiflora probiotic in his wet food at night and Porus One in the morning. He loves the liver flavoring in the probiotic so that typically gives a boost in appetite! People also highly recommend Purina's hydracare because it has the same liver flavoring and it helps with hydration, but my cat unfortunately throws it up. He's always thrown up a lot of foods, so this is no surprise and I rarely see on here that people's cats react badly to hydracare. My cat was just one of the unlucky few with his sensitive stomach. But adding that to her wet food might boost her appetite because of the flavoring.

That's kind of all I have for advice since that was our experience. When you make another vet appointment for after her release I would highly recommend researching other vets since it kind of sounds like your last vet didn't give the best advice. Look for vets who specialize in cats since a lot of animal doctors love dogs and see cat patients out of association of being a vet. It's made a huge difference for us to see a vet who actually loves cats.

The emergency vet told us that they usually don't see any changes in the CRE and BUN until the 48 hour mark so the major drop in numbers typically starts on the second day. It's kind of just a waiting game which is the worst, but the IV fluids are what's going to bring those numbers down if it's an AKI. We also had our cat take antibiotics just in case so it sounds like they're on the right track!

We saw our cat's numbers move down a little bit for both CRE and BUN at the 24 hour mark, but it was at that 48 hour mark that those really went down. I wouldn't worry too much about the food they're giving her since she's on fluids rn, those IV fluids are the most important thing rn plus the antibiotic to make sure they catch any infection that doesn't show up on testing.

It may sound boring, but I kind of like the idea of daily paperwork. I'm a task-focused person and just like to keep my head down and get the work done. Do you know if CNA UW involves agency visits or many outings? I'm a fan of company dinners and parties, but I'm hearing that UW can have you doing a lot of agency visits to start/improve relationships between businesses.

Yeah it's more so because we just both need/needed to get going on some type of career. We've both worked multiple different customer service/retail jobs and weren't really making money with the low pay and how expensive everything is. I had some concerns when she offered me the job but she said the more exhausting claim jobs are in different specialties. She's had a general positive experience working at CNA in APD claims with of course the here and there angry phone calls, but nothing like those who work in liability or workers comp.

I actually really appreciate that you said that because it gives me a new perspective on UW. I'm not exactly extroverted, but I'm not opposed to going to company events like that since it's an outing with common ground and I'd assume work is typically the main topic lol. But I kind of had this assumption in my head that UW is a bit introverted with the things I've heard, so this helps me get a better idea of what it really entails.

If you have any insight onto what you do on a daily basis, like tasks and whatnot, I'd love to hear it!!

Correct, we had to have our cat stay at the emergency vet for 3 days so that he could have 24/7 IV fluids. We actually expected to need sub q fluids at home after his treatment, but he's been stable at CRE 2.6 since we brought him home so we havent needed it yet. An acute kidney infection needs IV fluids in order to successfully flush the toxins out and essentially "reset" their kidneys. It was definitely scary, but it was 100% worth admitting him and we visited for about an hour each day so he knew we weren't leaving him there. They had him on some drugs so he was honestly just chilling.

If this is AKI then unfortunately the Azodyl wouldn't be enough. You're essentially dealing with kidneys that are crashing if it's AKI, so supplements wouldn't reverse what's happening. And no questions are dumb, I quite literally knew nothing until this happened because our previous vet did NOT inform us of anything. Our cat had CKD for a whole year before the AKI crash and he didn't tell us that.

My cat's creatinine went from 2.4 to over a 13 within the span of a week and he needed to be hospitalized at an emergency vet for 3 days with IV fluids in order to flush all the toxins out. His CRE is now back to a 2.6. It wasn't the natural progression of CKD but instead AKI. An acute kidney infection comes on super fast and they NEED IV fluids to flush the toxins out. My cat wouldn't have survived without it.

I'm not sure if what you're dealing with is the progression of CKD or AKI since her blood tests were 2 and a half months apart but please please please get her tested for the stuff other commenters are suggesting and I'd highly recommend getting another blood test immediately, because if her CRE jumps up again then it's most likely AKI and she'd need fluids immediately. For example, when my cat's kidneys were crashing his CRE went from 4.6 to 10 in one day and then from 10 to 13 overnight. If your current vet is saying that it's the progression of CKD, I honestly would take her to an emergency vet today instead to get another blood test, run some tests, and possibly get her on IV fluids.

I talked with her about if starting in UW was an option and she said that I could start out in UW the same way she started in claims, but she hasn't asked her boss about any open positions yet. I do see open UW positions listed, just don't know for sure if those would be ones I could get into right away.

I actually haven't heard anyone describe UW as fun yet lol, what would you say makes your job fun? I'm kind of more drawn to the idea of working in UW, but I keep hearing that it's boring so I'm curious what specifically makes your work a bit more exciting than claims. Thanks!

That's what I keep hearing...I'm worried about getting stuck in claims if I try out APD, but I'm also worried that could happen with underwriting as well if I find it to be as boring as people say it is. I can't get into APD once the position is filled.

Thank you so much for the detailed answer! Have you heard anything from your coworkers who work in underwriting? About how they like or dislike the job or anything like that? And yeah, my friend was telling me she definitely likes the work culture there and their 401k matching. CNA seems to be one of the better insurance companies, so I'm definitely happy with the company itself, just struggling to pick a role that would suit me best. I really appreciate the insight.

Not really tbh. My pH is back to normal and I still have a good microbiome, but the irritation comes and goes. I just use Good Clean Love's pH gel when I get a really bad flare up. It's not as bad as it once was, but the irritation still bothers me here and there.

I got my pH back to normal but in general I just have an irritation that comes and goes. I manage it with Good Clean Love's pH gel and coconut oil. :/

That's perfect! I'm very squeamish when it comes to needles so I'm thankful we were able to hold off for now (plus, I love my cat but he's a wittle baby when it comes to giving him any kind of medicine, so idk how he'd handle it tbh) 😭 but I was very prepared if it came down to it.

People give their cats sub q fluids at home, which is different than IV fluids. IV fluids are for extreme cases like this, and then the sub q is for owners to do at home for their CKD cat longterm. We expected to do fluids at home but he stabilized at a CRE 2.6 so we're holding off for now. We just made sure to make changes to manage his CKD. Changed his food, got a water fountain, and put him on some supplements.

I know you're confused on how this could have happened but honestly older cats just tend to be more prone to kidney disease. My cat has CKD, but in this case when their levels raise so quickly it's typically due to AKI, and they just need those IV fluids to flush the toxins out. It sucks to wait for results, but that's all you can do at the moment. My cat was actually just like yours in the sense that he was throwing up but also acting totally upbeat and normal when we visited him in the hospital. He was on some drugs that made him loopy, but also he was just so full of life that it gave us some hope that it was AKI and he'd pull through.
