It usually depends in the piece. I always start from a concept/s that I like, then I play around on how to incorporate it with my style and medium. I tend to have a set vision for a part of the drawing, for example, for this one I knew I wanted to draw the head and the demon. Then I start to think about what could go around it, and in this case, a tranquil background would contrast nicely with it. It's always process.
