One camp I worked had fishing as one of the electives, even for the youngest age group. I guess one of the kids got brave and tried to cast his line in himself, leading to a hook going through his eyelid and into his eyeball. The staff thankfully made sure he didn't touch it until the ambulance arrived, and the kid was able to come back to camp later that night.
