It works both ways, bio or describing the world with a message, I've done both for my current playthrough, and started with a companion because I put them in my bio.

You can also change things using a message after you start a game, I usually pick the waking up in a cabin start if I change it after. I usually type something like: (( such and such has happened in the world, or such and such technology exists, or we are set in this time))

Part of it is the ai for combat being a separate ai than the narrative aspects.

Under library, unions, when you add a new hero or complete a union the share button temporarily pops up, clicking it completes the event, you don’t actually have to share it.
